Acknowledgements and Waiver

I agree to and acknowledge the following:

1. I consent to participate in the activity of LOST in Perth at Level 1, 138 Murray Street, Perth 6000

2. I am aware that dim lighting, sound effects and other jump scare effects are used in the activity of LOST in Perth.

3. There are risks involved with participating in the escape room activity at LOST in Perth which I have read about in this form. I have considered the risks and I freely choose to assume responsibility for any risk including any risk that has not been identified through the reasonable efforts of LOST in Perth.

4. I waive any right to claim against the operator of the venue and their respective staff, the owners and employees of LOST in Perth and if applicable, owners and leasers of the premises on which the activity takes place, for any personal injury (or death) of the participant and for any loss or damage to property of the participant arising from the participant’s participation in the activity of LOST in Perth.

5. I will follow the rules of LOST in Perth and comply with reasonable directions as advised to me from time to time by LOST in Perth while at the site.

6. I agree to compensate the operator of the venue for all loss and damage to the set, equipment and any other property of LOST in Perth caused by the participant.
